Offset commission start and end dates with commission templates

When a commission template is selected in the install form, by default the start date is filled in with the installation date, and the end date field is left empty.
It would be useful to have the ability to specify a number of months that each of these two fields should be offset from the installation date.
That would support templates such as:
- Pay host 10$ for the first month
- Pay host 50% of hotspot rewards starting on the second month
